Nursing Home Transition Diversion (NHTD) Waiver
NHTD Waiver is available to individuals with disabilities and seniors to receive services in the most appropriate and least restrictive setting. The waiver may help people remain at home in the community. UCM provides nurse case management, Service coordination, and Care Transition to ensure that participants are returned to the community with all the services needed to remain safe at home.

The TBI Waiver is available to survivors of traumatic brain injury and offers: UCM provides service coordination, independent living skills training, counseling, home and community support services, environmental modifications, medical equipment and more. This program is designed to be a cost-effective alternative to nursing home placement which enables the participants to live in the least restrictive setting possible. The TBI waiver philosophy is to provide complete freedom of choice

Patient Review Instrument (PRI)
A Patient Review Instrument (PRI) is an assessment tool developed by the New York State Department of Health to assess selected physical, medical, and cognitive characteristics of nursing home residents, as well as to document selected services that they may receive. The PRI is basically a 'picture' of a patient's current medical, physical and cognitive status. PRI and Screen to enable them to determine a patient's condition for long term placement. UCM is available to go into the community to complete PRIs to assess the level of care of individuals
